La Descrizione
Recentemente, sono state sviluppate tecniche innovative con l’utilizzo di robot che permettono una maggiore precisione nel posizionamento delle componenti, elevata accuratezza e migliore risultati per i pazienti sia per la protesi monocompartimentale che per la protesi totale di ginocchio.
Il sistema robotizzato permette di personalizzare la procedura chirurgica sulle caratteristiche anatomiche del paziente, rispettando i tessuti molli e di conseguenza la cinematica del ginocchio. Questo comporta una maggiore accuratezza nel posizionamento dell’impianto protesico, un migliore allineamento dell’arto e un migliore bilanciamento legamentoso che si traduce in una maggiore durata dell’impianto e soddisfazione del paziente.
Tecnica chirurgica
Il robot è utilizzato per l’impianto della protesi totale, la protesi mono-compartimentale mediale e laterale, sia per la protesi della femoro-rotulea. L’intervento viene eseguito attraverso una piccola incisione chirurgica del ginocchio. I sensori per la navigazione robotizzata vengono applicati alla gamba e alla coscia.
Le superfici articolari vengono identificate con un apposito strumento che permette una ricostruzione 3D del ginocchio senza dover eseguire esami TC preoperatori. Il chirurgo procede quindi alla registrazione della tensione dei legamenti attraverso l’applicazione di forze al ginocchio lungo un arco di movimento di flessione e estensione. In base alla ricostruzione 3D e allo studio cinematico del ginocchio, il chirurgo è in grado di posizionare le componenti protesiche nella posizione ottimale. Il robot interviene nell’esecuzione dei tagli femorali e tibiali permettendo un posizionamento ottimale delle componenti protesiche con un margine di errore inferiore a 0.5 mm.
Il risultato
Il paziente è in grado di alzarsi e camminare dopo 4 ore dall’intervento, seguendo il protocollo Fast Track. La dimissione avviene in 2-5 giorni dall’intervento presso il domicilio o in struttura riabilitativa.
